Rather than waiting for TSH to rise above conventional cutoffs or focusing solely on hormone replacement, it prioritizes a full thyroid panel, aims for optimal rather than merely “in-range” values, and addresses the underlying drivers of autoimmunity. Hormone replacement (levothyroxine, combination T4/T3, or natural desiccated thyroid) remains important when needed, but it does not address the autoimmune process itself.
1. Reduce Inflammatory and Trigger Foods Many people with Hashimoto’s improve on an anti-inflammatory, nutrient-dense diet that eliminates common immune triggers. Strict gluten elimination is frequently recommended because of molecular mimicry between gluten proteins and thyroid tissue; studies and clinical experience show antibody reductions and symptom improvement in a substantial subset of patients. Other common eliminations include dairy, refined sugars, processed foods, and sometimes soy, legumes, or nightshades on a trial basis. Emphasizing whole foods, adequate protein, healthy fats, fiber, and colorful vegetables supports gut barrier integrity and lowers systemic inflammation. Some protocols use short-term lower-carbohydrate or Mediterranean-style patterns with reported antibody drops. Individual response varies—work with a practitioner and consider testing for celiac disease or food sensitivities.
2. Low-Dose Naltrexone (LDN) LDN (typically 0.5–4.5 mg at bedtime) is an off-label use of naltrexone that appears to modulate the immune system via temporary opioid receptor blockade, which upregulates endorphins and can reduce pro-inflammatory cytokines. Clinical observations and patient reports describe improvements in fatigue, pain, mood, sleep, and sometimes reductions in TPO/Tg antibodies. Evidence specifically for Hashimoto’s is limited (largely anecdotal, case series, and observational data rather than large randomized trials). One registry study found no clear reduction in thyroid hormone prescriptions after starting LDN. It is generally well-tolerated but requires compounding, medical supervision, and is not a first-line therapy. It may be considered as an adjunct when foundational steps (diet, nutrients, gut health) are already in place.
3. Targeted Nutritional Supplements Several nutrients have research support for lowering antibodies or supporting thyroid function:
Selenium (often 200 mcg/day as selenomethionine): Multiple studies and meta-analyses show significant reductions in TPO antibodies (sometimes 40% or more). It supports glutathione production and protects the thyroid from oxidative stress.
Myo-inositol + selenium: Combination therapy has shown benefits for TSH, Free T3/T4, and antibody levels in some trials.

Always test levels where possible and use supplements under professional guidance—more is not always better.
Additional Foundational Supports
Gut health is central: intestinal permeability (“leaky gut”), dysbiosis, and infections can perpetuate autoimmunity. Addressing these, managing stress (chronic cortisol elevation impairs conversion and raises reverse T3), optimizing sleep, and treating concurrent issues (blood sugar dysregulation, other autoimmune conditions) form the broader functional medicine framework.

A typical functional approach starts with a comprehensive panel and symptom review, removes major dietary triggers, repletes key nutrients, supports gut and stress resilience, optimizes thyroid hormone replacement if indicated, and considers adjuncts such as LDN when appropriate. Progress is monitored by both symptoms and serial labs (antibodies often take months to decline meaningfully).
